특발성 혈소판 감소성 자반증, Idiopathic thrombocytopenic purpura(ITP)

원인 

  • 골수 혈구모세포(줄기세포)→거대핵 모세포(거핵모세포)→거대핵세포(거핵세포)에서 발육된 후 순환혈액 내 혈소판이 된다. 
  • 대부분의 혈소판은 정상적으로 혈관 내 주로 있다. 
  • 혈액과 혈액응고 인자의 양과 기능도 정상적이고 혈관도 정상인데 혈관내 혈소판의 항원 항체 이상반응이 생겨 혈소판이 정상 이상 파괴되어 혈소판 수가 정상 이하로 감소될 수 있다. 
  • 자가면역 병으로 인해 생기는 혈소판 감소 병을 특발성 혈소판 감소성 자반증, 또는 원발성 혈소판 감소성 자반증이라고 한다.
  •  특발성 혈소판 감소성 자반증은 일종의 자가면역 병이다.
  • 이 병의 50%는 이 병이 발생되기 2~3주 전 감기나 그 외 다른 종류의 바이러스성 상기도염을 앓은 병력이 있다. 
  • 그래서 바이러스 감염이 이 병을 유발시키는 인자라고 추정하기도 한다. 
  • 그렇지만 아직도 원인을 확실히 모른다. 

증상 징후 

  • 이 병은 어느 연령층 아이들이나 성인들에게도 생길 수 있으나 10세가 되기 바로 전 학령기 아이들에게 더 잘 생길 수 있다. 
  • 감소된 혈소판 수치에 따라 증상이 다르다. 
  • 심할 때는 피부와 점막 등에 피가 맺히고 점상출혈 등 출혈 반점이 생길 수 있고 피부가 퍼렇게 멍들 수 있다. 
  • 이런 증상을 자반증이라고한다. 
  • 더 심할 때는 코피도 날 수 있고 두개강 내 출혈, 위장 관내 출혈, 방광 등 비뇨기 내 출혈 등이 생길 수 있다. 
  • 백혈병, 재생불량성 빈혈, 또는 외상 등으로 피부가 쉽게 멍들 수 있고 자반증이 생길 수 있다. 피부가 쉽게 멍이 들거나 코피가 자주 나면 특발성 혈소판 감소성 자반증이나 다른 종류의 출혈성 질환 등이 있나 알아보는 것이 좋다.

진단

  • 병력, 증상과 진찰소견 등을 종합해서 이 병이 의심되면 CBC 피검사를 하여 혈소판 수가 얼마인가 알아보고 혈소판의 모양 등을 검사하고 진단할 수 있다. 
  • 정상 혈소판 수는 150,000~400.000개인데 150,000개나 그 이하로 뚝 떨어질 수 있고 때로는 혈소판 수가 5,000~10,000 정도까지 비정상적으로 아주 낮게 떨어질 수 있다.
  • 때로는 골수 흡인검사를 해서 확진하는데 도움을 얻을 수 있다. 
  • 급성 백혈병, 재상불량성 빈혈, 또는 불길한 다른 혈액 병과 감별 진단하는 것이 보통이다. 
  • P 출혈 환자 스크린 검사(출혈 선별검사 ) Screening of the bleeding patient (Bleeding screening test) 

 치료

  • 이 병을 완치시킬 수 있는 특효약은 아직 없다. 
  • 사소한 외상으로 신체의 어느 부위에서든지 피가 날 수 있다. 그 때문에 외상을 입지 않도록 조심해야 한다. 특히 피부, 관절 내, 근육 내, 두개강 내, 복강 내 등 신체 각 부위에서 출혈이 생기지 않도록 주의해야 한다. 
  • 머리나 관절이나 복부 등 신체가 어디에 심하게 부딪치면 부딪친 부위에서 외 출혈이 생길 수 있고 그 체부 출혈의 정도에 따라  의료구급대원, 병원 응급실, 또는 의사의 지시에 따라 치료해야 한다. 
  • 특발성 혈소판 감소성 자반증이 경미할 때는 상처를 입지 않도록 예방적 치료를 해 주고 관찰적 치료를 하면 자연적으로 낫을 수 있다. 
  • 심할 때는 경구용 프레드니손으로 약 2주 동안 치료하거나 또는 혈소판 수가 정상으로 돌아갈 때까지 치료한다. 
  • 이 병의 75% 정도가 발병된 후 수주에서 3개월 이내에 자연히 치료될 수 있다. 
  • 이병의 88% 정도는 발병후 9~12개월 이내 완치되는 것이 보통이고 10%는 장기 치료가 요한다고 한다. 
  • 드물게  만성 특발성 혈소판 자반증은 비장 적출 수술로 치료한다. 
  • 2% 정도는 여러 가지 방법으로 치료해도 완치되지 않는다. 
  • 감마 글로불린 정맥주사로 치료하기도 한다. 또 항 D 글로불린으로 치료하기도 한다. 
  • Rituximab이란 약으로 치료하기도 한다. 

Idiopathic thrombocytopenic purpura (ITP)

Cause

• Bone marrow hemoblasts (stem cells) → megakaryocytic blasts (megakaryoblasts) → megakaryocytic cells (megakaryocytes) develop into platelets in the circulating blood.

• Most platelets are normally found mainly in blood vessels.

• Although the blood and blood clotting factors are normal and blood vessels are also normal, an adverse reaction with the antigen-antibody of platelets in the blood vessels may occur and platelets may be destroyed more than normal, resulting in a decrease in the platelet count below normal.

• Thrombocytopenic disease caused by autoimmune disease is called idiopathic thrombocytopenic purpura or primary thrombocytopenic purpura.

• Idiopathic thrombocytopenic purpura is an autoimmune disease.

• 50% of these patients have a history of a cold or other viral upper respiratory tract infection 2-3 weeks before the onset of the disease.

• Therefore, it is assumed that viral infection is the cause of this disease.

• But we still don’t know for sure.

Symptoms, Signs

• The disease can affect children and adults of any age but is more likely to occur in school-age children just before the age of 10.

• Symptoms vary depending on the decreased platelet count.

• In severe cases, blood may form on the skin and mucous membranes, bleeding spots such as petechiae may occur, and the skin may bruise blue.

• This symptom is called purpura.

• In more severe cases, nosebleeds may occur, intracranial hemorrhage, gastrointestinal bleeding, and urinary tract bleeding such as bladder may occur.

• Leukemia, aplastic anemia or trauma can cause the skin to bruise easily and cause purpura. If your skin bruises easily or you have frequent nosebleeds, it is good to check for idiopathic thrombocytopenic purpura or other types of bleeding disorders.

• If this disease is suspected by combining the medical history, symptoms, and examination findings, a CBC blood test can be performed to determine the platelet count, and the shape of platelets can be examined and diagnosed.

• The normal platelet count is 150,000 to 400.000, but it can drop to 150,000 or less, and sometimes the platelet count can drop abnormally very low, as high as 5,000 to 10,000.

• Sometimes a bone marrow aspiration test can help confirm the diagnosis.

• Differential diagnosis from acute leukemia, aplastic anemia, or other ominous blood disorders is common.

• P Screening of the bleeding patient (Bleeding screening test) cure

• There is no specific drug that can cure this disease yet.

• Minor trauma can cause bleeding anywhere on the body. That is why you have to be careful not to injure yourself. In particular, care must be taken not to cause bleeding in each part of the body, such as the skin, intra-articular, intramuscular, intracranial, and abdominal cavity.

• If the body is hit hard, such as the head, joint, or abdomen, external bleeding may occur from the bumped area. Depending on the degree of body bleeding, treatment should be performed according to the instructions of a medical paramedic, hospital emergency room, or doctor.

• When idiopathic thrombocytopenic purpura is mild, preventive treatment is given to avoid injury, and observational treatment can result in spontaneous recovery.

• In severe cases, treatment with oral prednisone for about 2 weeks or until platelet count returns to normal.

• About 75% of the disease can be cured spontaneously within a few weeks to 3 months after onset.

• About 88% of morbidity cases are usually cured within 9 to 12 months after onset, and 10% require long-term treatment.

• Rarely, chronic idiopathic thrombocytopenic purpura is treated with splenectomy.

• About 2% are not cured even with various treatments.

• It is sometimes treated with intravenous gamma globulin. It is also treated with anti-D globulin.

• It is sometimes treated with a drug called Rituximab.
