천식(기관지 천식) 발작 기전, Asthma attack(Bronchial asthma attack) pathogenesis

 

천식(기관지 천식)의 발작 기전 (그림 1~8)

기도(Airways)가 외부에서 들어온 유발인자신체 내부 유발인자에 노출되면 다음 1~8과 같은 병변이 기도에 생기고 그에 따른 증상 징후가 공통적으로 생길 때 천식이라고 할 수 있다

  • 천식 발작이 유발되는 기전은 어느 정도 잘 알려졌지만 아직도 확실히 모르는 점도 많다.
  • 천식을 앓을 수 있는 유전인자를 타고날 때부터 지니고 있는 소아들이 천식 발작 유발 인자에 노출될 때 그 유발인자에 의해 기관지 벽에 있는 평활근이 수축되고 기도 부분이 비정상적으로 수축되고 좁아진다.
  • 이런 현상을 기관지 수축 또는 기도 수축이라 한다.
  • 천식 발작이 유발되면, 기관지가 수축되는 동시에 기관지 벽이 붓고 그 벽에 염증이 생기고 기관지 속에 점액이 분비되고 그 결과로 기관지 속이 더 좁아지게 된다. [부모도 반의사가 되어야한다-소아가정간호백과]-제 8권 소아청소년 호흡기 질환-만성 기관지염 참조

1. 천식(기관지 천식)의 발작의 전체적 기전

  • 육체적 운동이나 그 외 다른 종류의 천식 발작 유발인자로 기도가 수축할 수 있고 기도 일부가 막힐 수 있다.
  • 이때 기관지 확장제로 치료하면 정상적으로 기도가 되돌아갈 수 있다.
  • 천식발작이 유발되면 기도염이 생길 수 있고 기도가 부을 수 있지만 천식 증상 징후가 거의 나타나지 않을 수 있고 때로는 나타날 수 있다.
  • 이미 천식발작이 유발인자로 생겼을 때 기도염이 생긴데다가 거기에 또 다른 천식 발작 유발인자에 또 노출되면 기도염이 더 심해질 수 있고, 기도가 더 붓고 점액분비가 더 증가되고 결과적으로 기도가 더 막힐 수 있다.  Used with permission from Hardy 1992

4. 경증도 천식으로 생긴 기도 수축

5. 심한 천식으로 생긴 기도의 수축

6. 경증도 천식 발작으로 생긴 기도 수축

7. 경증도 천식 발작

③ 중등도 지속 천식 발작이 유발될 때 기관지와 세기관지 부분기도가 더 많이 수축되고 기도 속이 더 좁아지고 기도 내벽이 더 붓는다.

④ 중증 지속 천식 발작이 유발될 때는 기관지와 세기관지 부분 기도가 더 많이 수축되어 그 기도 속이 더 좁아지고 그 기도 내벽이 더 붓고 염증이 더 심하게 생길 수 있다.

8. 천식 발작 기전

a-정상 기도(기관지),

b-급성 기관지 수축,

c-만성 천식으로 기도가 수축되고 기관지 벽에 염증이 생  겨 기도가 더 좁아진 것,

d-아 급성 천식이나 만성 천식으로 기관지 벽이 붓고 거기에 염증이 생겨 기도가 더 좁아졌다.

e-염증성 세포, 

m-비만세포에서 히스타민 과립체가 분비된다. 

T 세포,

B 세포, 

p 폐포 대식 세포, 

o 호산구, 호중구 등이 활성화되어 생긴 히스타민, 사이토카인, 류코트리엔, 프로스타글란딘 등 여러 가지 매개체가 생겨 그것들이 복합적으로 작용해 천식이 발작될 수 있다.

Attack( mechanism of asthma (bronchial asthma)

(Figures 1-8) When the airways are exposed to an external trigger or a trigger inside the body, the following lesions 1 to 8 occur in the airways and the symptoms and signs are common. Seizure mechanism of asthma (bronchial asthma) as seen in pictures (Figures 1-8)

• Mechanisms that trigger asthma attacks are somewhat well known, but there are still many unknowns.

• When children who are born with a genetic factor for developing asthma are exposed to an asthma attack trigger, the trigger causes the smooth muscle in the bronchial wall to contract and the airway area to contract and narrow abnormally.

• This phenomenon is called bronchoconstriction or airway constriction. • When an asthma attack is triggered, the bronchi contract and at the same time the walls of the bronchial tubes become inflamed and inflamed, and mucus is secreted into the bronchi, resulting in a narrower bronchial cavity. • Physical exercise or other types of asthma triggers can constrict the airways and block parts of the airways.

• If treated with a bronchodilator at this time, the airway can return to normal.

• When an asthma attack is triggered, you may develop airway inflammation and your airways may become swollen, but you may have few and sometimes no signs of asthma symptoms. • If you have airway inflammation when an asthma attack is already a trigger, and you are also exposed to another trigger for an asthma attack, your airway may become more severe, with more airway swelling and increased mucus secretion, and consequently more airways. can be blocked Used with permission from Hardy 1992

7. Mild asthma attacks

③ When a moderate persistent asthma attack is triggered, the bronchial and bronchiolar partial airways constrict more, the airway becomes narrower, and the airway lining becomes more swollen.

④ When a severe and persistent asthma attack is triggered, the bronchial and bronchiolar airways constrict more, making the airway narrower, and the airway lining may become more swollen and inflamed. 8. Asthma Attack Mechanism
