전신 홍반성 낭창(전신 홍반성 루프스)의 개요

• 전신 홍반성 낭창은 항원 항체 이상반응으로 전신의 여러 계통의 여러 기관과 조직에 염증성 질환이 생기는 병이다.
• 특히 전신에 있는 혈관에 염증이 생긴다. 
• 전신 홍반성 낭창이 있으면, 병명 그대로 전신의 각 계통의 각 장기에 여러 종류의 병변이 생길 수 있다.
• 신장에는 급성 사구체신염 또는 만성 사구체신염이 생길 수 있고, 
• 그 외 다른 형태의 신장 병변이 생길 수 있다. 
• 피부에는 홍반성 낭창(홍반성 루프스)이 생길 수 있다. 

전신 홍반성 낭창(전신 홍반성 루프스)에 의한 사구체신염의 증상 징후

• 전신 홍반성 낭창으로 생긴 신장의 병변의 정도에 따라, 또는 신체의 여러 계통의 장기에 생긴 홍반성 낭창의 병변의 주증도에 따라 이 병의 증상 징후가 다르다. 
• 현미경적 혈뇨가 있을 수 있고, 소변 화학 검사에서 피가 조금 나올 수 있으며, 육안으로 쉽게 볼 수 있을 정도 피가 소변에 많이 나올 수 있다. 
• 동맥경화증, 고혈압, 혈 중 고혈당증, 전신부종, 단백뇨, 혈중 전해질 불균형 등이 생길 수 있다. 
• 심하면, 만성 사구체신염으로 진행될 수 있고, 그로 인해 신부전증이 생길 수 있다. 

전신 홍반성 낭창(전신 홍반성 루프스)에 의한 사구체신염의 진단

• 병력·증상 징후·진찰소견 등을 종합해서 이 병이 의심되면, 피 검사와 소변검사 등을 통해 진단할 수 있다. 
• 신장 생체 조직 검사로 이 병을 확진할 수 있다. 

전신 홍반성 낭창(전신 홍반성 루프스)에 의한 사구체신염의 치료

• 프레드니손(Prednisone), 아자지오프린(Azathioprine), 사이클로포스파마이드(Cyclophosphamide),Benlysta(bilimumab) 등으로 치료한다.
• 심한 신부전증은 투석 치료나 신장이식 수술로 치료한다. 
• 최근에는 Mycophenolate mofetil로 치료하기도 한다.

Overview of systemic lupus erythematosus

• Systemic lupus erythematosus is a disease that causes inflammatory diseases in various organs and tissues of various systems throughout the body due to an antigen-antibody adverse reaction.

• Inflammation occurs, especially in blood vessels throughout the body.

• With systemic lupus erythematosus, as the name suggests, various types of lesions can occur in each organ of each system throughout the body.

• The kidneys may develop acute glomerulonephritis or chronic glomerulonephritis;

• Other types of kidney lesions may occur.

• The skin may develop lupus erythematosus (lupus erythematosus).

Symptoms, signs of glomerulonephritis caused by systemic lupus erythematosus 

• Symptoms and signs of this disease differ depending on the severity of the kidney lesion caused by systemic lupus erythematosus or the severity of the lesion of various organs in the body.

• There may be microscopic hematuria, there may be little blood in the urine chemistry test, and there may be a lot of blood in the urine that is easily visible to the naked eye.

• Arteriosclerosis, high blood pressure, hyperglycemia, systemic edema, proteinuria, and blood electrolyte imbalance may occur.

• In severe cases, it can progress to chronic glomerulonephritis, which can lead to kidney failure.

Diagnosis of glomerulonephritis due to systemic lupus erythematosus 

• If the disease is suspected based on the medical history, symptoms, signs, and examination findings, it can be diagnosed through blood tests and urine tests.

• Kidney biopsy can confirm the disease.

Treatment of glomerulonephritis due to systemic lupus erythematosus 

• Treatment with Prednisone, Azathioprine, Benlysta(belimumab), or Cyclophosphamide.

• Severe renal failure is treated with dialysis or kidney transplantation.

• Recently, it has been treated with Mycophenolate mofetil.
