  • 우유 알레르기를 우유 알러지 또는 우유 단백 알레르기라고도 한다. 우유에 포함된 우유 단백으로 유유 알레르기가 유발될 수 있다.
  • 우유에 든 유당으로 유당 불내증이 생길 수 있다. 유당 불내증은 우유 알레르기가 아니다. 우유를 마실 때 생우유 속에 든 세균 감염으로 감염성 질환에 걸릴 수 있고, 우유 속에 든 페니실린 등 항생제로 항생제 알레르기도 유발될 수 있다.
  • 우유 알레르기에는 심한 즉시 우유 알레르기와 경한 일시적 우유 알레르기 두 가지 형이 있다.
  • 심한 우유 알레르기는 IgE 항체 항원 반응으로 유발되는 우유 알레르기이고
  • 경한 일시적 우유 알레르기는 IgE 항체 항원 반응으로 생기지 않는 우유 알레르기이다.
  • 우유 단백 알레르기의 발생 빈도는 얼마나 되는지 정확히 모른다.
  • 0.3~7% 정도로 추정한다.
  • 최근 연구에 의하면, 우유 알레르기 발생 빈도가 증가 추세에 있다.
  • 소아청소년 소화기계 질환 전문의의 치료를 받으러 온 소아 위장관 질환 환자들의 30%가 우유 단백 알레르기를 앓고 있었다고 한다(참조문헌-Pediatric News, March 2004).

우유 알레르기(우유 알러지)의 증상 징후

  • 1세 전 영아들의 1.5~3.5%에게 우유 단백 알레르기가 생길 수 있다.
  • 우유 알레르기가 생후 2일~4개월에 처음 생길 수 있다.
  • 그러나 돌 전후에 가장 잘 생긴다고 한다. 그 이유는 확실히 모르나, 위장관의 점막층에 IgA항체가 충분히 존재하지 않기 때문에 우유 알레르기가 생길 수 있다.
  • 알레르기 질환의 병력이 있거나 아토피 체질이 있는 친 부모 형제자매들의 형제자매들 중 누군가에게 우유 알레르기가 유발될 확률이 더 높다. 영아기에 생긴 우유 알레르기는 2~3세 사이에 자연적으로 없어질 수 있다.
  • 우유 알레르기는 영유아들의 만성 설사와 구토의 주원인이 되고 위장관 만성 흡수 불량증의 원인이 되기도 한다.
  • 체중감소, 저단백질 혈증, 빈혈, 부종, 성장지연, 만성 흡수 불량증이 우유 알레르기로 생길 수 있다.
  • 구토, 복통, 설사, 점액 변과 혈 변, 위식도 역류, 영아산통, 입술과 혀의 부종, 코 막힘, 비강 점막층 충혈, 천식, 아토피 피부염, 두드러기, 기저귀 피부염, 아나필락시스 반응, 맥관 신경성 부종, 과다 활동장애 및, 또는 집중력 결여증도 우유 알레르기로 인해 생길 수 있다.
  • 여기서 언급되지 않은 외에 다른 여러 증상 징후 또 병이 우유 알레르기로 인해 생길 수 있다.

우유 알레르기(우유 알러지)의 진단

  • 가족 알레르기 병력과 환아 자신의 과거의 병력과 현재의 병력, 증상 및 징후, 진찰소견, 우유 단백 IgE 항체(면역 글로불린 E 항체) 혈중 농도 검사, 알레르기 피부 시험, 유발 검사 등의 결과를 총 종합해서 우유 알레르기를 진단할 수 있다.
  • 우유에 알레르기가 있으면 우유 단백 IgE 항체 혈중 농도가 증가되어 있는 것이 보통이다.
  • 자녀들에게 우유 알레르기가 생길 가능성은 친부모 형제자매 중 한 사람이 아토피 체질이면 25%이고, 부모 둘 다 아토피 체질이면 40~50%이고, 두 부모가 같은 종류의 알레르기 질환을 갖고 있을 때는 70~80%이다.
  • 다음 유발 검사의 예와 같이 우유 알레르기를 유발 검사로 진단할 수 있다.
    우유 알레르기 진단, 우유 단백질 알레르기로 생기는 설사 참조
    • 우유나 우유가 든 음식물, 또는 우유 단백이 들은 음식물을 2~4주 동안, 또는 증상 및, 또는 징후가 다 없어질 때까지 섭취하지 않는다.
    • 그 대신 인공영양을 먹는 영아에게는 우유 성분이 없는 저 알레르기 인공영양을 2주 정도 먹인다.
    • 우유 단백 성분이 있는 인공영양을 먹지 않기 시작한 72시간 내 우유 알레르기의 증상 징후가 없어지는 것이 보통이다.
    • 우유 단백이 들지 않은 음식물을 먹지 않기 시작한 후 2~4주 동안 우유 알레르기로 생기는 증상 징후가 덜 하는지, 아주 다 없어지는지, 더 심해지는지, 그 증상 징후가 조금도 없어지지 않고 그대로 남아 있는지 관찰한다.
    • 2~4주 관찰하는 동안 그 증상 징후가 하나도 없거나 아주 덜하면 우유 단백이 들은 인공영양 15cc를 저 알레르기 인공영양 240cc에 섞은 후 나이에 적절한 양의 인공영양을 첫날 하루 동안 먹인다.
    • ‘이때 아무런 증상 징후가 생기지 않으면 우유 단백이 들은 같은 종류의 인공영양 30cc를 저 알레르기 인공영양 240cc에 섞어 나이에 적절한 양을 먹인다.
    • 증상 징후가 없으면 유발 검사 시작 3일째 되는 날에는 우유 단백이 들은 인공영양 45cc를 저 알레르기 인공영양 240cc에 섞어 먹인다.
    • 이때도 증상 징후가 없으면 그 후 4~5일간 같은 방법으로 먹인다.
    • 유발 검사를 할 때 우유 알레르기가 있는 아이는 우유 단백이 들은 인공영양을 다시 먹기 시작한 후 72시간 내에 우유 알레르기로 인한 증상 징후가 다시 나타나는 것이 보통이다.
    • 전에 먹었던 우유 단백이 들은 인공영양을 다시 먹는 동안 전과 같은 또는 거의 같은 우유 알레르기의 증상 징후가 또다시 나타날 때는 우유 알레르기가 있을 가능성이 많다’고 추정 진단할 수 있고 그 증상 징후가 나타나지 않을 때는 우유 알레르기가 ‘없을 가능성이 많다’고 추정 진단할 수 있다.
    • 만약 유발 검사를 하는 동안 우유 알레르기의 증상 징후가 경미하게 생겨 우유 알레르기가 있는지 없는지 확실히 모를 때는 유발 검사를 한 번 더 반복하든지 우유 피부 시험 및또는 우유 단백 IgE 항체(면역 글로불린 E 항체) 혈액 농도 검사 등을 추가로 해서 우유 알레르기가 있나 확실히 진단할 수 있다.
    • 아나필락시스 반응 등 심한 우유 알레르기가 있을 때는 우유를 한 방울이라도 먹여서는 안 된다. 부모가 임의의 유발 검사로 우유 알레르기를 진단해서도 안 된다.
    • 유발 검사는 원칙적으로 진보적 심폐소생술을 즉시 시작할 수 있는 의료시설이 갖추어 있는 병원이나 응급실에서 의사의 지시에 따라 또는 의사가 직접 하는 것이 원칙이다. 그리고 유발 검사를 받는 아이나 그의 부모가 이런 검사를 하는지 알지 못하게 인공영양을 먹이면서 유발 검사를 하는 것이 원칙이다.
    • 유발 검사의 결과가 양성으로 나타나서 우유에 알레르기가 있을 가능성이 많이 있어도 피부 반응 시험을 해서 우유 단백에 알레르기가 있나 재확인하는 것이 보통이다
    • 우유 알레르기가 있을 것 같아서 우유나 우유가 든 음식물이나 우유 단백이 들은 음식물을 조금도 주지 않고 관찰하는 2~4주 동안 우유 알레르기의 증상 징후와 비슷한 증상 징후가 계속 있든지 그 증상 징후가 더 심해질 때는 우유 알레르기가 있을 가능성을 완전히 배제할 수는 없지만, 우유 알레르기 이외 다른 병으로 우유 알레르기로 인해 생긴 증상 징후와 비슷한 증상 징후가 생겼다고 오진할 수 있다.
    • 요즘 흔히 사먹을 수 있는 많은 종류의 음식물 성분 속에 우유나 우유 단백이 다소 섞여 있을 수 있다. 우유 알레르기를 유발 검사로 진단할 때는 전 우유, 생우유, 또는 우유 단백이 들은 인공영양은 물론 극소량의 우유나 우유 단백이 들은 음식물을 먹어서는 안 된다.
    • 한 가지 항원에 의해 어떤 종류의 알레르기 질환이 생긴 아이들은 다른 종류의 항원에도 알레르기 질환이 생길 가능성이 많다.
    • 아토피 체질이 있는 아이들이나 알레르기 비염, 기관지 천식, 두드러기, 약물 알레르기, 그밖에 다른 알레르기 질환을 가지고 있는 아이들에게는 우유 알레르기가 더 잘 생기기 쉽다.
    • 친부모 형제자매들 중 누구에게 우유 알레르기가 있는지, 다른 항원으로 알레르기 질환이 있는지 가족 병력을 잘 알면 우유 알레르기를 진단하는데 많은 도움이 되며 우유 알레르기는 대를 이어 체 염색체 우성으로 유전되는 것이 보통이다.
    • 우유 알레르기가 있으면 달걀, 생선, 땅콩 등 음식물에도 식품 알레르기가 생길 가능성이 더 높다.
    • 피부 시험 및, 또는 RAST 등은 진단하는데 도움이 될 수 있다.
    • 특히 IgE 항체 항원 반응으로 생기는 심한 우유 알레르기를 유발 검사로 진단할 때 심한 우유 알레르기가 생길 가능성이 있다.
    • 우유 알레르기가 심하게 생기면 즉시 치료할 수 있는 진보 심폐소생술이 가능한 의사의 관찰 하에서만 유발 검사를 해야 한다.
    • 심한 우유 알레르기가 있는 아이는 에피네프린 키트나 아나키트, 또는 에피펜 주사를 항시 지참하거나 적절한 곳에 보관하여 필요할 때 즉시 쓸 수 있도록 한다.

  • 우유에 알레르기가 있는 영아들은 우유 단백이 들은 인공영양을 먹을 수 없고 콩 단백으로 만든 인공영양을 먹을 수 있다.
  • 그러나 IgE 항원 항체 반응으로 심한 우유 알레르기가 생긴 병력을 가진 영아들의 7~13%는 콩 단백으로 만든 인공영양에도 알레르기가 생길 수 있다.
  • 우유 알레르기로 생긴 장염이 있는 영아들의 20~40%가 콩 단백으로도 장염이 생긴다.
  • 영아들의 우유 알레르기 장염의 증상의 30~40%는 우유 알레르기가 시작한 후 1~2년 동안 계속되는 것이 보통이다.
  • 경한 우유 알레르기가 있는 영유아들에게 콩 단백이 들은 인공영양, 수육 단백질 인공영양, 부분적 가수분해로 만든 단백질 인공영양이나 광범위 가수분해 단백질로 만든 저 알레르기 인공영양, 또는 유리 아미노산 인공영양 중 하나를 선택해서 먹일 수 있다.
  • IgE 항원 항체 반응으로 생긴 심한 우유 알레르기가 있는 영아들에게는 광범위 가수분해 단백질로 만든 인공영양이나 유리 아미노산 인공영양을 먹여야 한다. 그렇지 않으면 모유를 먹여야 한다.
  • 콩 단백질로 만든 인공영양이나 광범위 가수분해 단백질로 만든 인공영양을 먹여도 증상 징후가 계속되면 유리 아미노산 인공영양을 먹여야 한다.
  • 이런 특수 인공영양의 값이 상당하고 맛이 좋지 않은 것도 단점이다.
  • 친부모 형제자매들에게 알레르기 병력이 많고 신생아에게 우유 알레르기가 생길 가능성이 있을 때는 모유 수유를 하면 여러 면으로 좋다.
  • 수유모는 우유, 생선, 땅콩 등 식품 알레르기를 잘 유발시킬 수 있는 음식물을 수유하는 동안 섭취해서는 안 된다.
  • 우유 알레르기가 생길 가능성이 많은 영아들에게는 사정상 모유를 먹일 수 없을 때는 부분 가수분해 단백질로 만든 저 알레르기 인공영양이나 광범위 가수분해 단백질로 만든 저 알레르기 인공영양(특수 인공영양)을 처음부터 먹여야 한다.
  • 이유식도 될 수 있는 한 생후 6개월 이후부터 먹이기 시작하고 우유는 적어도 첫 돌 전에는 먹이지 말고, 달걀은 적어도 2세 전에, 생선, 땅콩과 다른 종류의 견과류는 3세 전에 먹여서는 안 된다.
  • 우유 알레르기를 진단받은 후 적어도 12~18개월 동안 의사의 지시 없이 우유를 먹여서는 안 된다.

애기가 우유를 먹고 트림을 시키거나 트림을 하지 않으면 5-10분쯤 안고 있다가 눕히면 우유를 조금 토해 내거나 입으로 흘러나옵니다.

그리고 종종 코로도 나오는 경우가 있습니다.

며칠 전 아기가 코가 막혀 가까운 소아청소년과에 들렀다고 코로 우유가 나온다고 했더니 구강 구조에는 이상이 없다고 하셨어요.

어른들 말씀으로는 나올 때 양수를 먹은 애들이 그렇다고 하던데 사실인가요??

그리고 애기가 감기는 아닌 것 같은데 (열은 없음코가 막힙니다.

특히 새벽에 조금 더 심한 것 같구요.

신생아라 약을 먹이기가 좀 망설여지는데 좋은 방법이 없을까요?

아기가 토하는 것과 젖을 넘기는 것은 서로 다른 증상입니다.

토하는 것은 위장관의 어느 부분이 막히거나 위장염이 있거나뇌에 어떤 이상이 있거나 또는 그 외 다른 이유로 먹은 위장관 내용물이나 위장관 속 장액 등이 입 밖으로 힘차게 나오는 증상입니다.

먹은 음식물이 힘없이 입안이나 비강 속으로 나오는 것을 위식도 역류 또는 일류라고 하며 이 두 가지의 증상을 확실히 구별하기가 때로는 어렵습니다.

과식을 하거나 너무 자주 먹거나 분유가 체질에 맞지 않거나 위식도 역류가 있을 때 젖을 넘기거나 토하는 경우가 가장 흔합니다.

위식도 역류에는 생리적 위식도 역류와 병적 위식도 역류가 있습니다그 원인에 따라 물론 치료를 해야 합니다.

• Milk allergy is also called milk allergy or milk protein allergy. Milk protein in milk can cause milk allergy.

• Lactose intolerance can result from lactose in milk. Lactose intolerance is not a milk allergy. When drinking milk, bacterial infections in raw milk can lead to infectious diseases, and antibiotic allergies such as penicillin in milk can also be triggered.

• There are two types of milk allergy: severe immediate milk allergy and mild transient milk allergy.

• Severe milk allergy is a milk allergy caused by an IgE antibody antigen reaction.

• Mild transient milk allergy is a milk allergy that does not result from a reaction to an IgE antibody antigen.

• You do not know exactly how often milk protein allergies occur.

• It is estimated at 0.3~7%.

• According to recent studies, the incidence of milk allergy is on the rise.

• It is reported that 30% of pediatric gastrointestinal disease patients who came to see a child and adolescent gastrointestinal disease specialist had milk protein allergies (Reference-Pediatric News, March 2004).

Symptoms, signs of a milk allergy

• 1.5% to 3.5% of infants before the age of 1 may develop a milk protein allergy. • Milk allergies may first develop in the 2nd to 4th month of life.

• However, it is said to be best seen before and after the stone. The reason for this is not clear, but milk allergy can occur because there are not enough IgA antibodies in the mucous layer of the gastrointestinal tract.

• Someone of the siblings of parental siblings with a history of allergic disease or atopic constitution is more likely to develop a milk allergy. Milk allergies in infancy can go away spontaneously between 2 and 3 years of age.

• Milk allergy is a major cause of chronic diarrhea and vomiting in infants and young children, and may also cause chronic malabsorption of the gastrointestinal tract.

• Weight loss, hypoproteinemia, anemia, swelling, growth retardation, and chronic malabsorption can be caused by a milk allergy.

• Vomiting, abdominal pain, diarrhea, mucous stool and bloody stool, gastroesophageal reflux, infantile colic, swelling of the lips and tongue, nasal congestion, nasal mucosa, asthma, atopic dermatitis, urticaria, diaper dermatitis, anaphylactic reaction, vascular edema, Hyperactivity disorder and/or lack of concentration can also be caused by a milk allergy.

• A number of other symptomatic signs and illnesses other than those not mentioned here can be caused by a milk allergy. table.

Diagnosis of milk allergy (milk allergy)

• The likelihood of developing milk allergies in children is 25% if one of the siblings of the birth parent has an atopic constitution, 40-50% if both parents have an atopic constitution, and 70~ when both parents have the same type of allergic disease. 80%.

o If there are no signs of symptoms at this time, feed in the same way for 4 to 5 days after that.

o When performing a provocation test, it is common for children with milk allergies to reappear symptoms of milk allergy within 72 hours after starting to re-eat artificial nutrients containing milk protein.

o If the same or almost the same symptoms of milk allergy as before while re-eating an artificial nutrient containing the milk protein that you had previously eaten, the milk allergy can be presumed to be’presumably’ and when the symptoms do not appear. o In principle, the provoking test is in principle performed at the hospital or emergency room equipped with medical facilities that can start progressive CPR immediately, according to the doctor’s instructions or by the doctor himself. And it is a rule to conduct a provocation test while feeding artificial nutrients so that the child or his or her parents who are undergoing provoking tests do not know whether they are doing such tests.

o Even if the result of the trigger test is positive and there is a high possibility that you are allergic to milk, it is common to reconfirm whether you are allergic to milk protein by performing a skin reaction test.

o If you are likely to have a milk allergy and you continue to have symptoms similar to those of a milk allergy for 2 to 4 weeks of observing without giving any milk or foods containing milk or foods containing milk protein, or if the symptoms become more severe. The possibility of a milk allergy cannot be completely ruled out, but it is possible to misdiagnose that a disease other than a milk allergy has produced symptoms similar to those caused by a milk allergy.

o Milk or milk protein may be somewhat mixed in many kinds of food ingredients that are commonly available these days. When diagnosing milk allergy as a provoking test, you should not eat whole milk, raw milk, or artificial nutrition containing milk protein, as well as foods containing very small amounts of milk or milk protein.

o Children who develop some type of allergic disease from one antigen are more likely to develop allergic disease from another type of antigen.

o Children with atopic constitution, allergic rhinitis, bronchial asthma, hives, drug allergies, and other allergic diseases are more susceptible to milk allergy.

o Knowing which of the siblings of the biological parents has milk allergy and family history of allergic diseases with other antigens is very helpful in diagnosing milk allergies, and milk allergy is usually inherited by somatic chromosomal dominance from generation to generation.

o If you have a milk allergy, you are more likely to develop a food allergy to foods such as eggs, fish, and peanuts. o Skin tests and, or RAST, etc. may be helpful in diagnosing.

o There is a possibility of severe milk allergy, especially when diagnosing a severe milk allergy caused by an IgE antibody antigen reaction by a provocation test.

o If a milk allergy develops severely, provoking tests should be performed only under the observation of a doctor who has advanced CPR that can be treated immediately.

o Children with severe milk allergies should always bring the epinephrine kit, Anakit, or EpiPen injection with them, or keep them in an appropriate place so they can be used immediately when needed.

• However, 7 to 13% of infants with a history of severe milk allergy due to IgE antigen antibody reactions may also develop allergies to artificial nutrition made with soy protein.

• 20 to 40% of infants with enteritis caused by milk allergy also develop enteritis from soy protein.

• 30-40% of symptoms of milk allergy enteritis in infants usually persist for 1 to 2 years after the onset of milk allergy.

• For infants and toddlers with mild milk allergies, choose one of: artificial nutrition with soy protein, artificial nutrition with meat protein, artificial nutrition with partially hydrolyzed protein or hypoallergenic artificial nutrition with broadly hydrolyzed protein, or artificial nutrition with free amino acids. So you can feed it.

• Infants with severe milk allergies resulting from IgE antigen-antibody reactions should be fed artificial nutrition made with broadly hydrolyzed proteins or artificial nutrition with free amino acids. Otherwise, you will have to breastfeed.

• Infants who are most likely to develop milk allergies should be fed low-allergic artificial nutrition made of partially hydrolyzed protein or low-allergic artificial nutrition made of broadly hydrolyzed protein (special artificial nutrition) from the beginning when breastfeeding is not possible due to circumstances.

The more information you know about your child’s age, gender, past medical history, family medical history, medical examination findings, and clinical examination, the more helpful it is to give you an answer. We will respond based on the information you have given. You said milk, but milk means formula.

Vomiting a baby and passing milk are different symptoms. Vomiting is a symptom of a blockage of the gastrointestinal tract, gastroenteritis, brain abnormalities, or other reasons, such as the contents of the gastrointestinal tract or the fluid in the gastrointestinal tract coming out of the mouth. Gastroesophageal reflux or gastroesophageal regurgitation is when food eaten out of the mouth or nasal cavity is called gastroesophageal reflux, and it is sometimes difficult to clearly differentiate between the two symptoms. It is most common to overeat, eat too often, when the formula doesn’t fit your constitution, or when you have gastroesophageal reflux.

There are two types of gastroesophageal reflux and physiological gastroesophageal reflux. Depending on the cause, of course, treatment must be performed.

Frequent stuffy nose can also be caused by allergic rhinitis or vasomotor rhinitis. If you combine the symptoms of passing the milk and the frequent clogging of the nose, you might wonder if the milk protein in the formula is suitable for the baby’s constitution.
