  • 벌에 쏘여 벌 알레르기 반응이 생기면 벌독 알레르기(Bee venom allergy), 또는 벌 알레르기라고 한다.
  • 미국에서는 벌독 알레르기로 연간 40명이 죽는다.
  • 한국에서도 벌 알레르기로 죽었다는 뉴스를 과거 몇십 년 동안 가끔 듣고 읽었다.
  • 아나필랙시스 반응의 가장 흔한 원인은 페니실린이고 그 다음으로 가장 흔한 원인이 벌독 알레르기이다(출처; Clinical seminar by Allen Lapey, MD).
  • 벌에 쏘이면 크게 세 가지 벌독 반응이 유발될 수 있다.
    • 하나는 벌독 국소 반응이고
    • 다른 하나는 전신 벌독 알레르기 반응이고,
    • 셋째는 지연성 벌독 반응인 혈청병이다.
  • 전신 증상 증후가 나타나지 않고 벌에 쏘인 국소가 조금 붓고 거기가 아프고 가려운 증상이 생기는 반응을 벌독 국소 반응이라고 한다(사진 5-3 참조).
  • 벌독 항원과 벌 항체의 항원 항체 이상 반응으로 특이적 면역 반응이 전신에 생기는 반응을 전신 벌독 알레르기 반응이라 한다.
  • 전신벌독 알레르기 반응은 일종의 아나필락시스 반응이다.
  • 벌에 쏘여 벌독 국소 반응만 생겼을 때는 벌독 알레르기라고 하지 않는다.
  • 벌에 쏘인 후 쏘인 국소가 아프고 붓고 붉어지고, 잠시 후 쏘인 국소에 가려운 증상이 생기는 것이 보통이다.
  • 드물게 벌에 쏘인 국소뿐만 아니라 그쪽 팔이나 다리의 상당한 부분이 많이 붓고, 또 그쪽 팔이나 다리 전체가 심하게 붓고 아플 수 있다. 이 경우도 일종의 벌독 국소 반응에 속한다.
  • 벌독 국소 반응이 있을 때는 혈압이 떨어지지도 않고 호흡곤란도 생기지 않고 두드러기도 나지 않는다.

  • 위에서 이미 설명한 바와 같이, 벌에 쏘여 벌 항원 항체 이상 반응으로 특이 면역 반응이 전신에 생길 때 전신 벌독 알레르기라고 한다.
  • 벌독 알레르기 반응이 생길 때는 다음과 같은 증상 징후를 동반한 아나필락시스 반응이  유발될 수 있다.
  1. 벌에 쏘인 후 15분 이내에 생기는 것이 보통이고,
  2. 전신에 두드러기가 나고,
  3. 얼굴이 홍조되거나 창백해지고,
  4. 혈압이 떨어지며,
  5. 호흡곤란이 생기는 것이 보통이다.
  6. 쏘인 국소가 가렵고 심하게 붓고,
  7. 전신이 가렵고,
  8. 상기도의 점막층이 붓고,
  9. 목안이 조이고,
  10. 어지럽고,
  11. 말하는 데도 곤란을 느낄 수 있다.
  12. 심할 때는 쇼크에 빠지고 사망할 수 있다.
  13. 벌에 쏘여 벌 알레르기가 생길 때 앞서 설명한 여러 가지 증상 징후들 중 한두 가지 증상 징후만 나타날 수 있다.
  14. 어떤 때는 벌 알레르기의 증상 징후가 경미하게 나타날 수 있다.
  15. 한 종류의 벌에 쏘여 벌독 알레르기가 생겼던 아이나 성인이 다른 종류의 벌에 또 다시 쏘였을 때도 벌독 알레르기가 또 유발될 수 있다.
  16. 드물게는 벌에 쏘인 후 혈청병이 유발될 수 있고 신증후군이 유발될 수 있다.

  • 병력 증상, 검진결과 등을 종합해 진단한다.
  • 벌 몸통 전체에서 적출한 순수 단백질로 만든 항원이나 벌독으로 만든 항원으로 피부검사를 해서 벌 알레르기가 있는지 없는지 진단할 수 있다.
  • 과거에 한 번도 벌에 쏘인 적이 없는 아이가 벌에 처음 쏘일 때도 벌 알레르기가 유발될 수 있고,
  • 과거에 벌에 쏘인 후 벌 알레르기가 생겼던 병력이 있던 아이가 그 후 벌에 또 다시 쏘일 때 벌 알레르기 반응이 유발될 수 있다. 그런 아이에게 벌 알레르기로 아나필락시스 반응이 생길 가능성이 많이 있기 때문에 에피네프린(Epinephrine)성분이 든 주사약(상품명 ‘에피펜 자가 주사기 주사약’ 또는 아나 자가 주사기 주사약)으로 응급 치료를 해 주는 것이 이상적 응급 치료방법이다.
  • 즉시 의료 구급대를 전화로 부르고, 병원 응급실, 또는 단골 소아청소년과 의사의 지시에 따라 현장에서 즉시 치료를 시작해야 한다. 그리고 이런 응급 처치를 한 후 가장 가까운 병원 응급실로 이송하여 추적 응급 치료를 받아야 한다.
  • 전신 벌 알레르기가 있거나 벌에 쏘여 벌독 알레르기가 유발될 가능성이 있는 아이 자신이나 성인은 필요할 때 응급 치료를 할 수 있는 에피네프린 주사약(상품명 ‘아나키트 자가 주사기 주사약’ 또는 ‘에피펜 자가 주사기 주사약’)을 집과 학교와 승용차 안에 항상 보관하고 있다가 벌에 쏘이면 환아 자신이 자기에게 직접 주사 놓아 치료하거나 주위 사람이 즉시 환아에게 주사를 놓아 치료해야 한다.
  • 병원 응급실에서는 벌 알레르기의 중증도와 증상 징후에 따라 에피네프린제, 코르티코스테로이드제, 항히스타민제, 베나드릴제, 아미노필린제, 전해질용액 정맥 주사, 혈압 상승제, 산소 호흡 치료로 치료한다.
  • 벌에 쏘여 국소 반응이 생길 때는 벌에 쏘인 부위를 얼음물 주머니 찬찜질로 치료한다.
  • 벌에 쏘인 국소가 아프거나 가려우면 베나드릴이나 아타락스제 등 경구용 항히스타민제로 치료한다.
  • 때로는 코르티코스테로이드제 연고나 경구용 코르티코스테로이드제로 치료한다.

벌 알레르기(벌독 알레르기) 예방

  • 옥외에서 음식물을 먹을 때 벌들이 잘 모여들 수 있고 벌에 쏘이기 쉽다.
  • 벌에 알레르기가 있는 아이가 집 밖에서 음식물을 먹을 때는 벌에 쏘이지 않도록 특별히 주의한다.
  • 전신 벌 알레르기 병력이 있는 아이는 꽃이 만발하는 꽃밭이나 벌들이 많이 모이는 곳에 가지 않도록 하며 벌들을 모이게 할 수 있는 향수나 화장품을 쓰지 말아야 한다.
  • 벌에 쏘인 후, 벌독 주머니 속에 든 벌독이 몸속으로 다 주입되는 데는 2~3분 정도 걸리는 것이 보통이다.
  • 벌독 주머니에 달린 독침이 피부에 그대로 박혀있을 때는 조심히 족집게로 독침을 잡고 뺀다. 피부에 박힌 독침을 뺄 때 벌독 주머니를 누르면 벌독 주머니 속 벌독이 몸속으로 다 주입될 수 있기 때문이다.
  • 에피펜 자가 주사기 주사약이나 아나키트 자가 주사기 주사약은 에피네프린제 성분이 들어있는 아나필락시스 반응 응급치료용 상품명이다.
  • 에피펜 자가 주사기 주사약이나 아나키트 자가주사기 주사약은 지참하기가 간편하고 아나필락시스 반응을 응급으로 치료할 필요가 있을 때 환아 본인이나 주위에 있는 다른 사람이 허벅다리에 쉽게 주사를 놓을 수 있게 만든 에피네프린제 주사약이다.
  • 에피펜(EpiPen) 자가 주사기 주사약에는
  • EpiPen 2-Pak 자가 주사기 주사약과
  • EpiPen Jr 2-Pak 자가 주사기 주사약 두 종류가 있고 조금 다르게 포장됐다. 이 두 종류의 EpiPen 자가주사기 주사약에 든 Epinephrine 용량도 다르다.
  • 의사로부터 처방받아 놓았다가 필요할 때마다 쓴다.
  • 실제로 쓰기 전 사용법을 미리 배워야 한다.
  • 벌 알레르기가 있는 아이가 사는 집, 학교, 승용차, 또는 자주 가는 곳에 아나키트(에피펜 자가 주사기 주사약) 하나를 보관했다가 벌에 쏘일 때 응급용으로 쓴다. 그리고 가능하면 본인이 그 주사약을 지참해야 한다.

  • 면역 요법은 약 3~5년 동안 하는 것이 보통이다.
  • 면역 요법 비용은 비교적 많은 편이다.

• A bee sting allergic reaction is called a bee venom allergy, or bee allergy.

• In the United States, bee venom allergy kills 40 people a year.

• In Korea, over the past few decades, I have occasionally heard and read the news that he died of bee allergy

• The most common cause of anaphylactic reactions is penicillin, followed by bee venom allergy (source; Clinical seminar by Allen Lapey, MD).

• A bee sting can cause three major bee venom reactions.

• o One is a local reaction to bee venom

• A reaction in which the bee sting is slightly swollen, painful, and itchy without systemic symptoms is called a local bee venom reaction (see Photo 5-3).

• A systemic bee venom allergic reaction is a reaction in which a specific immune reaction occurs throughout the body due to an adverse reaction between the bee venom antigen and the antigen-antibody of the bee antibody.

• Systemic bee venom allergic reaction is a type of anaphylactic reaction.

• When a bee sting causes only a local reaction to bee venom, it is not called a bee venom allergy.

• After a bee sting, it is common for the sting to become painful, swollen and red, and after a while the sting is itchy.

• In rare cases, not only the localized bee sting but also a significant part of the arm or leg may be swollen, and the arm or leg may be severely swollen and painful. This case also belongs to a kind of local bee venom reaction.

• When there is a local reaction of bee venom, there is no drop in blood pressure, no breathing difficulties, and no hives.

12. In severe cases, you may fall into shock and die.

13. When a bee sting causes a bee allergy, only one or two of the symptoms described above may appear.

14. Sometimes the symptoms of bee allergy may be mild.

15. A child or adult who has developed a bee venom allergy from a sting of one type of bee may develop a venom allergy again when stung by a different type of bee again.

Diagnosis and treatment of bee allergy (bee venom allergy)

• Diagnosis is made by synthesizing medical history, symptoms, and examination results.

• It is possible to diagnose whether there is a bee allergy by skin testing with an antigen made from pure protein extracted from the whole bee body or antigen made from bee venom.

• A child who has never been stung by a bee in the past may develop a bee allergy when it is sting for the first time;

• A child who has had a history of bee allergy after being stung by a bee may develop a bee allergic reaction when he is subsequently stung by a bee again. Since there is a high possibility that such a child may have an anaphylactic reaction due to bee allergy, it is an ideal emergency treatment method to give emergency treatment with an injection containing epinephrine (brand name ‘Epipen auto-injector injection’ or ANA auto-injector injection).

• Immediately call the medical emergency department by phone, start treatment immediately at the hospital emergency room, or on the spot as directed by your regular pediatrician. After performing such first aid, the patient should be transported to the nearest hospital emergency room for follow-up emergency treatment.

• Children and adults who have a systemic bee allergy or who may be allergic to bee venom from bee stings should receive epinephrine injection (trade name: ‘Anakit Auto-Injector Injection’ or ‘Epi-Pen Auto-Injector Injection’) for emergency treatment when necessary. If it is always kept at home, in school, or in a car, and is stung by a bee, the patient must either inject himself or herself to receive treatment, or have someone around him or her immediately inject and treat the affected child.

• In the hospital emergency room, treatment with epinephrine, corticosteroids, antihistamines, benadryl, aminophylline, electrolyte solution, intravenous injection, blood pressure increaser, and oxygen breathing therapy, depending on the severity and symptom signs of bee allergy.

• In case of a local reaction from a bee sting, treat the area of ​​the bee sting with a cold pack of ice water. • If the bee sting is painful or itchy, treat it with an oral antihistamine such as Benadryl or Atarax.

• Sometimes treated with corticosteroid ointments or oral corticosteroids.

Prevention of bee allergy (bee venom allergy)

• When eating outdoors, bees can gather easily and are prone to bee stings.

• Children with a history of systemic bee allergy should not go to flower gardens or places where there are many bees and should not use perfumes or cosmetics that can attract bees.

• If the stinger in the bee venom bag is still embedded in the skin, carefully grab the stinger with tweezers and pull it out. This is because if you press the bee venom bag when removing the stinger from the skin, the bee venom in the bee venom bag can be injected into the body.

• Epipen Auto-Injector Injection or Anakit Auto-Injector Injection is a brand name for emergency treatment of anaphylactic reactions containing epinephrine.
